1) We require you to fill out a Police Vetting form.

2) Send the form to us at: Info@surrogategrandparents.org.nz along with a copy of your photo ID (driving licence or passport preferred)
3) Complete your donation via Internet Banking to .

4) Once we receive your clear Police vetting form back - you are then a member of Surrogate Grandparents and we will get to work to find you a suitable match.
